As promised, attached is a copy of the Vermont Buprenorphine Guidelines as written with assistance from local and national sources. I hope that you find these guidelines helpful in your provision of services to opiate dependent patients who might benefit from agonist treatment.  For your convenience, we have included links to particular pages on the SAMHSA website for exemption forms as well as a copy of the PATH form that needs to be completed prior to prescribing buprenorphine for patients with Medicaid
