Dear Colleague,

 

As promised, attached is a copy of the Vermont Buprenorphine Guidelines as written with assistance from local and national sources. I hope that you find these guidelines helpful in your provision of services to opiate dependent patients who might benefit from agonist treatment.  For your convenience, we have included links to particular pages on the SAMHSA website for exemption forms as well as a copy of the PATH form that needs to be completed prior to prescribing buprenorphine for patients with Medicaid.

 

If the past six months of national policy changes regarding buprenorphine is any indication of things to come, it would mean that this set of guidelines may need to be updated on close to a weekly basis. Please forgive ADAP for not rising to that challenge! Please check on the SAMHSA web site for regular updates, especially in the FAQs section, the most recent of which is included here. The guidelines will be reviewed on a yearly basis and are being distributed nationally, so we expect a great deal of feedback.

 

Our office will be sending out bulletins as new and important information about buprenorphine and agonist treatment in general are available.
